Job SummaryProvides direct and indirect patient care in the office setting to patients, families and caregivers in the Medical Group practice. -Uses the nursing process consistent with acceptable standards of the profession and MVHS Medical Group. -Provides high quality patient centered care that reflects initiative, flexibility and responsibility with a minimum amount of administrative supervision. -Determines priorities of care based on physical and psychosocial needs, as well as factors influencing patient flow through the system. -Communicates with outpatient clinic physicians and NP/PAs (healthcare providers) about changes in patient's status, symptomatology and results of diagnostic studies. -Is able to respond quickly and accurately to changes in condition or response to treatmentCore Job Responsibilities--- - - -Support the PCP in the delivery of care by gathering and documenting relevant clinical data, as appropriate and per patient need.
--- - - -Participate in the daily huddle to identify individual patient needs for upcoming appointments.
--- - - -Clinical liaison between patient and provider for questions, concerns and testing results. -
--- - - -Communicate effectively with other members of the care team in coordinating the individualized plan of care.
--- - - -Support PCP during in office procedures and examinations.
--- - - -Provide outreach and education to patients in need of services as identified through quality improvement initiatives (Population Health Management).
--- - - -Perform duties within scope of practice to the highest level of licensure.
--- - - -Provide direct support & guidance to other members of the care team as appropriate.
--- - - -Able to perform assessment data collection on all patients utilizing tools / forms in EMR.
--- - - -Adequately assess and reassess pain. -Educate the patient and family regarding pain management.
--- - - -Provide age appropriate care to patients in the community setting.
--- - - -Consistently document procedures completed in the EMR, to include medications administered and follow-up assessments
--- - - -Deliver medication utilizing the five (5) rights and observes patient for desired untoward effects.
--- - - -Maintain current knowledge of medications and their correct administration.
--- - - -Establish priorities of patient care based on needs and office resource
--- - - -Demonstrates the ability to assist providers with procedures and perform services requiring technical and manual skill
--- - - -Perform other duties as required.
-Education/Experience Requirements--- - - -Graduate of an accredited school of nursing or nursing program.
--- - - -Current New York State Registered Nursing registration.
--- - - -BLS Certification.
--- - - -Minimum one year of nursing experience
--- - - -Basic computer and EMR knowledge.
--- - - -Excellent communication and interpersonal skills.
--- - - -Ability to assimilate information from many different sources.
--- - - -Must be able to communicate in English both verbally and in writing. -
-Licensure/Certification Requirements--- - - -BSN from accredited nursing program -
--- - - -Experience in a specialty medical office.
--- - - -Certifications applicable to the assigned office's specialty.
